Start Application

Thank you for your interest in our Acute and Critical Care OT Fellowship.  Please carefully review the following application requirements checklist before submitting your application.

Application Requirements:

  • Proof of a Bachelor of Science, Master’s (MOT or MSOT) or Doctoral degree in Occupational Therapy (entry-level OTD or Post-Professional OTD), or unofficial academic transcript demonstrating eligibility to graduate from an ACOTE or WFOT occupational therapy academic program
  • Proof of a valid Tennessee Occupational Therapy license. Clinicians licensed in another state may submit their license information and new graduates may submit an unofficial academic transcript and/or NBCOT certification letter to demonstrate eligibility
  • Two letters of recommendation from healthcare professionals who can speak to moral and ethical character. At least one letter must be from a current or former occupational therapy supervisor, or fieldwork educator if the applicant is a recent graduate with less than 6 months experience. We strongly recommend that letters are submitted by individuals who can comment on academic and clinical abilities. Family member’s letter of recommendation will not be accepted
  • A letter of intent which includes the following components:
    • Introduction
    • Interest in acute care
    • Career and academic goals
    • Strengths and qualifications
    • Commitment to the fellowship
    • Concluding statement
  • Proof of an active AOTA membership available on the My AOTA Profile page
  • Current Curriculum Vitae

To initiate the application process, please click on the Start Application link to submit the online form. After submitting the initial application form, email [email protected] a single PDF document of the compiled application requirements to complete your application process. The application submission deadline is May 31, and final decisions will be delivered by July.

This program is currently seeking candidacy status for accreditation through the American Occupational Therapy Association (AOTA). While we are pursuing accreditation, please be advised that if the program does not receive AOTA accreditation by the time you complete your fellowship, you will not receive a certificate of completion from an AOTA-accredited fellowship program.
