Thank you for your interest in our Acute and Critical Care OT Fellowship. Please carefully review the following application requirements checklist before submitting your application.
Application requirements:
- Academic transcript or degree from an ACOTE or WFOT Occupational Therapy program
- Proof of a valid license, or be eligible for licensure, to practice occupational therapy in Tennessee
- Proof of CPR/BLS certification
- Two letters of recommendation from a healthcare professional who can speak to your moral and ethical character. At least one must be from a current or former occupational therapy supervisor, or fieldwork educator if a recent graduate with less than 6 months experience. We strongly suggest that you include individuals who can comment on your academic and clinical abilities. A family member’s letter of recommendation will not be accepted.

- Letter of intent which includes the following components:
- Introduction
- Interest in acute care
- Career and academic goals
- Strengths and qualifications
- Commitment to the fellowship
- Concluding statement
The above listed requirements are to be compiled into a single PDF document and emailed as an attachment to [email protected]. The application submission deadline is May 31, and final decisions will be delivered by July.
This program is currently seeking candidacy status for accreditation through the American Occupational Therapy Association (AOTA). While we are pursuing accreditation, please be advised that if the program does not receive AOTA accreditation by the time you complete your fellowship, you will not receive a certificate of completion from an AOTA-accredited fellowship program.
