TPS Data Security Incident

Erlanger is required to notify affected individuals of improper access, use, or disclosure of patient information, called Protected Health Information (PHI). On December 11, 2025, TriZetto Provider Solutions (“TPS”), notified Erlanger of a breach. TPS is a third-party vendor used by Erlanger for billing related services.

Report of incident as relayed by TPS: On October 2, 2025, TPS discovered suspicious activity in their systems. TPS quickly took steps to secure their systems and launched an investigation to determine the nature and scope of the activity. The investigation determined that there was unauthorized access to the TPS systems between November 2024 and October 2025, and that certain records related to insurance eligibility transactions were viewed from the TPS system. As a result, TPS began an extensive review of these records to determine what information they contained and to which TPS client the information belonged.

TPS informed Erlanger that they implemented additional security measures and reviewed existing security policies to further protect against similar incidents moving forward. TPS confirmed that data belonging to Erlanger Heath patients was included in the information potentially compromised. Approximately 219 individuals were affected. Information potentially compromised for patients and primary insureds includes name, birthdate, address, health insurance member number (which for some individuals may be a Medicare beneficiary number), health insurer name, primary insured information, dependent information. The Social Security Number for a smaller number of individuals might have been compromised. Importantly, not every affected individual had all of these data elements impacted, or the same combination of data elements impacted. Erlanger has no evidence of identity theft or fraud as a result of this incident. This incident has been reported to law enforcement and regulators.
