What you need to include in your request for compliance review

In order to review a situation to determine whether a compliance violation has occurred, the request must contain as much detailed specific information as possible.  You may remain anonymous if you would like but the anonymous request is the most difficult to review. The reason the anonymous request is so difficult to review is due to the fact that the Office of Compliance can not contact the individual filing the request to obtain additional information.
