Erlanger Hospital Quality Indicators

Leapfrog Group Quality Index of Safety Standards

This quality measure is made up of individual scores on 30 healthcare practices. Erlanger has fully implemented this measure—meaning the hospital is in the highest quartiles for both overall score and real progress in the four most heavily weighted practices. These practices are summarized below:

This measure requires a hospital to promote a culture of safety and to hire adequate nursing staff.
Communication Among Healthcare Workers
Fully implemented
This measure includes repeating verbal orders, using standardized abbreviations and doses, using original sources to write patient care summaries, giving patient care orders to all healthcare providers and correctly labeling X-rays.
Doctor and Patient Communications
Fully implemented
This measure requires that doctors discuss and agree on procedures with patients and that a patient's preferences for end-of-life care are displayed in his or her chart.
Prevent Infections
Fully implemented
This measure requires that care providers wash their hands with a disinfectant before and after seeing patients and that appropriate measures are taken to prevent patients from aspirating, developing infections related to having a catheter, assessing and prevent the risk of infections as a result of surgery and vaccinating healthcare workers to prevent flu infections.  
Prevent Medication Errors
Fully implemented
This measure requires that a pharmacist review all medication orders, an orderly work space is maintained for dispensing medications, medications are properly labeled and stored, high risk medications are identified and safeguarded, medications are purchase and stored in single use form and that safe processes are put in place for using anti-blood-clotting medication.  
Prevent Complications and Mistakes

This measure requires that physicians provide beta blockers to high-risk patients and the entire healthcare team takes adequate measures to prevent pressure ulcers and the occurrence of wrong-patient or wrong-site procedures, reduce the risk of renal injury from x-rays, and assess and prevent the risks of blood clots, malnutrition, and complications from tourniquets
