The Supplemental Life Insurance Plan is a voluntary, employee-paid group term
life insurance plan in which you can choose to participate to supplement your
employer provided life insurance.
Eligibility To be eligible, you must
be a regular employee assigned to work at least 19.5 hours per week (0.52-1.0
FTE) and have completed your six-month probationary period by January 1.
Enrolling Erlanger Health System
employees may enroll during the open enrollment period.
Benefit
Amount Employees may
purchase term life insurance coverage for one times their base annual
salary.
Cost Employees may purchase term
life insurance coverage for one times their base annual salary at a monthly rate
of 22 cents per $1,000 of coverage.
Effective Date To be eligible, you
must be a regular employee assigned to work at least 19.5 hours per week

Beneficiary Your beneficiary designation is kept in your personnel
file. You can change your beneficiary at any time by completing a new
beneficiary designation. The change is effective the date that it is received by
Erlanger. If you name
more than one beneficiary, they will share equally unless you provide
otherwise.
If a beneficiary dies before you, his or her share will be paid equally to
the surviving beneficiaries, unless you state otherwise. Any amount for which a
beneficiary is not named will be paid to your estate.
Funding The benefit is completely paid for by the
participants of the plan by the amounts determined by the insurance
company.
