Family/Medical/Maternity Leave

Leave of Absence
Erlanger Health System recognizes the fact that employees will, from time to time request leaves of absence from the Health System. Specific questions not addressed here may be directed to Employee Relations, in the Human Resource Department.

To be eligible for Family, Medical or Maternity leave under The Family and Medical Leave Act of 1993, an employee must be employed for 1 year and have worked 1250 hours during the past 12 months.
To be eligible for Erlanger’s Family, Medical or Maternity leaves, an employee must have completed their 6-month probation period.
Eligibility under the Family and Medical Leave Act of 1993 will be determined first. If the employee does not meet the requirements, eligibility will then be determined for Erlanger’s Family, Medical and Maternity leaves.

Qualifying Events
Events that may qualify for Family, Medical or Maternity leave are, but not limited to, the list below.
• Birth of a child
• Placement of a child for adoption or foster care
• Employee’s own serious health condition
• Serious health condition affecting, spouse, child or parent

Reinstatement Rights
Eligible employees have reinstatement rights during the first 120 days of Family, Medical or Maternity leave, whether it is under the Family and Medical Leave Act of 1993 or Erlanger’s Family, Medical or Maternity leave. Reinstatement rights for EHS temporary employees will not apply beyond the completion date of the temporary work assignment. The maximum leave time available is 12 months.

Pay during Leave
Employees taking a Family, Medical or Maternity leave may use accrued PAL or take the leave as unpaid. EIB is available for leave if the employee is out for his or her own illness. EIB is also available for female employees on Maternity leave during the first 6-weeks of the leave. EIB may be taken for a longer period of time if there were complications during the Maternity leave.

How to Apply
To apply for a Family, Medical or maternity leave, follow the steps below.

  1. Pick up a Leave Packet from Human Resources.
  2. Complete the “Application for Leave of Absence”, include approximate beginning date and approximate ending date. The employee and the manager must sign this form.
  3. Have the treating physician complete the “Medical Certification Form.”
  4. Turn the application and Medical Certification form in to the manager.
  5. Management will submit the request to Human Resource.
  6. Approval or Denial Letter will be mailed to employee by Human Resource.

If the leave is foreseeable, a 30-day notice is required. If the leave is not foreseeable, the leave application and medical certification must be submitted within 15 days of receipt of the leave packet.
