Erlanger offers term life insurance for eligible employees through
Unum. This employer-paid coverage is equal to one times an employee’s
annual base pay. Supplemental Term Life Insurance is a voluntary
employee-paid plan that allows you to purchase additional coverage equal to one
or two times your annual base pay.
Employees may purchase term life insurance coverage
for one or two times their annual base salary at a monthly rate of 22 cents per
$1,000 of coverage.
How do I enroll?
If you meet the eligibility criteria,
you are eligible to enroll during the Annual Open enrollment. To enroll in the
supplemental term life insurance, you must be a regular employee hired to work a
minimum of 19.5 hours per week (0.52 – 1.00 FTE) and have completed your 6-month
probationary period prior to January 1. You may increase your coverage by
an additional one or two times your annual salary by completing the appropriate
form with evidence of insurability (i.e., you will be required to answer several
medical questions and may be denied coverage by the insurer based on the
